Ghrelin Agonists: The Ultimate Offseason Mass Builder?

For an effective offseason, putting on quality muscle mass is essential, however it's not as easy as it seems. Doctors are constantly looking for modern day pharmacological drugs to keep patients anabolic and stimulate appetite, for example patients undergoing chemotherapy or patients with kidney disorders often suffer cachexia or muscle wasting. The most interesting pharmacological drug to date for increasing appetite is Ghrelin and Ghrelin agonists which maybe the perfect drug for the offseason bodybuilder with a poor appetite. Many bodybuilders may not feel hungry after resistance exercise which may be due to decreases in Ghrelin, in fact a recent study reported that Ghrelin was significantly depressed during and after an intense bout of resistance exercise, which may explain why many bodybuilders have no appetite after exercise8.

Ghrelin is a peptide hormone produced by the stomach and also by a variety of other tissues that is secreted predominantly by the stomach in response to fasting1. The sum effect of ghrelin is an increase in appetite and food intake, which, when coupled with ghrelin-mediated anti-inflammatory activity, have led to the current interest in the use of this agent in human cachexia or counteracting human wasting disorders, but bodybuilders looking to ramp up their appetite may also want to look into this drug. A previous study reported that ghrelin is a potent releaser of GH in normal subjects with a dose dependent pattern. In fact, Ghrelin is one of the most potent GH secretagogue's to date. Ghrelin has recently been shown to enhance appetite and improve quality of life in patients with wasting disorders. Here are a few studies of the potential benefits of Ghrelin for increasing appetite and enhancing lean body mass:

a.) One study examined 7 cancer patients with impaired appetite; they were administered a single dose of synthetic human ghrelin or placebo in a randomized crossover design. A significant increase (+31%) in the consumption of calories from an ad libitum or free feeding buffet meal offered immediately after ghrelin infusion was documented3.

b.) One study with adults with multiple cancer types and cachexia who received either 2 or 8 µg of human ghrelin/kg as a 60-min infusion before lunch on 2 study days, 7 days apart, in a randomized, placebo-controlled, double-blind, double crossover trial. Ad libitum meal food intake tended to improve during ghrelin admination4.

c.) In a randomized, double-blind, crossover study, 9 patients with dialysis-dependent chronic kidney disease and protein-energy malnutrition a single subcutaneous dose of ghrelin or placebo, followed by provision of a buffet meal. Administration of ghrelin resulted in a marked increase in food consumption meal energy intake (204% of placebo), and energy intake remained 57% higher in the ghrelin period than in the placebo period5.

d.) Human's given Ghrelin (2 µg/kg twice daily intravenously) for 3 weeks to cachexic patients with COPD in an open-label study. After ghrelin therapy, significant increases from baseline measures were observed for body weight (+1 kg or +2%), LBM (+0.7 kg or +1.8%), food intake (+9%), and hand grip strength6.

e.) In another unblinded protocol, this investigative group administered human synthetic ghrelin (2µ/kg of bodyweight twice daily intravenously) to 10 patients with congestive heart failure for 3 weeks. Ghrelin increased lean body mass and significantly improved hand-grip strength, left ventricular ejection fraction, workload, and peak oxygen consumption during cycle ergometry exercise; these parameters remained unchanged in 8 patients who did not receive ghrelin7.

Ghrelin Agonists...MK-677 and Amamorelin: Orally Active Ghrelin Secreatagogue's

There has recently been a approved orally administered ghrelin receptor agonist. Very limited data on the effects of ghrelin receptor agonists are published; however the studies have shown positive impacts on appetite stimulation. In a pilot, randomized, double-blind, placebo controlled, dose-escalation phase I study, researchers studied an orally available ghrelin mimetic called MK-677 in healthy volunteers. Results showed that the agonist produced a dose-related increase in body weight without dose-limiting adverse effects9. These authors also recently published data from a double-blind, randomized, placebo-controlled study that evaluated the short-term effects of this agent (anamorelin). Anamorelin was well tolerated, significantly increased blood GH concentrations at all doses, and sustained increases in blood IGF-1 concentrations compared with placebo after 5–6 d of treatment. Significant dose-related increases in body weight occurred that were directly correlated with changes in IGF-1 concentrations10. In a pilot double-blind trial in cachectic patients with multiple cancer types, oral anamorelin (50 mg/d for 12 wk) significantly increased LBM by1.3% compared with placebo and was well tolerated11. Finally, in a study published in the Annuals of Internal Medicine, The ghrelin mimetic MK-677 enhanced pulsatile GH secretion; daily MK-677 significantly increased GH and IGF-I levels to those of healthy young adults without serious adverse effects and significantly increased lean mass over 12 months12. Although there are only a few studies, the research is strongly suggests that appetite can be enhanced by the use of orally active secretagogue's.

The average person thinks eating 3-4 times per day with a diet consisting of burgers, fries and sodas is enough to fuel muscular growth. But this is far from the truth. Bodybuilding is a very simple formula – eat smart, train hard, rest and stay consistent. Stop whining about being a hardgainer and instead ENGORGE yourself!

For packing on thick, massive muscle, consuming large amounts of clean calories are key. If you're not eating enough calories, you WILL NOT grow. Reducing your calories is great for dieting down, but if you're looking to add SIZE, you have to go in the opposite direction and increase your caloric intake. This may be difficult, but after 2 weeks of eating more than you're used to, it becomes almost second nature. You should aim for at least 20-25 calories per pound of bodyweight. So if you weigh 200 lbs, you should strive to consume a minimum of 4,000-5,000 calories per day to get significantly bigger. If you have a really fast metabolism, you may require as much as 25-35 calories per pound of bodyweight. This may sound like a lot (especially when you consider it should be healthy protein, carbs and fats, and NOT junk food), but that's where ENGORGE comes into play.
